Tooth movement: Orthodontic treatment works by gradually shifting your teeth into their ideal positions. This movement continues throughout your entire treatment period. If we applied a gem while your teeth are still moving, the gem's position would shift along with your tooth. These attachments are strategically placed according to your treatment plan. A tooth gem would occupy space on the tooth surface, potentially interfering with these attachments or limiting where they can be placed.

Changing tooth positions: Just like with braces, your teeth are moving throughout Invisalign treatment. A gem applied at the start of treatment won't be in the same position relative to your tooth by the time treatment ends. You'll likely want to have it repositioned anyway, so waiting until treatment is complete makes more sense.

If you're in your final aligner trays and your orthodontist confirms your teeth have reached their final positions, we can sometimes make an exception — but even then, I generally recommend waiting until you're fully finished and in the retention phase.

The Ideal Timing: When to Book Your Gem

So when exactly is the right time to get a tooth gem after orthodontic treatment? Here's the timeline I recommend based on over 600 client treatments:

2-4 weeks after braces removal: Once your braces come off, your teeth need a short adjustment period. Your orthodontist will give you retainers to maintain your new tooth positions, and you'll need time for any residual bonding material to be cleaned off and your enamel to stabilise. I typically suggest waiting 2-4 weeks after your braces are removed before applying a gem.

After the initial retainer phase stabilises: If you're completing Invisalign treatment, you'll transition from active aligners to retainers. These retainers hold your teeth in their new positions and prevent them from shifting back. Once you're in the retention phase and your orthodontist confirms your teeth have stabilised, you're ready for a gem.

When you can consistently wear your retainer: Since you'll need to continue wearing retainers (typically at night) even after getting a gem, make sure you're in a good retainer-wearing routine first. Your retainers should fit comfortably and you should be wearing them as prescribed before we add a gem. If your retainers are the removable type, a small gem usually won't affect the fit, but we'll check this during your first appointment.

What About Permanent Retainers?

Many people finish orthodontic treatment with a permanent retainer — a thin wire bonded to the back of their front teeth to prevent shifting. The good news is that permanent retainers don't affect tooth gem application at all.
